Chronic Disease Management

Targeting diabetes, hypertension, high cholesterol, and obesityDo you suffer from diabetes, hypertension, high cholesterol, or obesity? If so, then the Chronic Disease Management program was designed just for you.

The Chronic Disease Management program combines nutrition, exercise and behavioral change to minimize the risks and complications of these increasingly common chronic diseases.

The program focuses on a multidisciplinary approach to manage these diseases and includes:

A health screening by a nurse
Ongoing review of data by an MD
A nutritional component
Individualized patient education

Some PROHEALTH programs may be covered by insurance, please call to determine eligibility and/or standard pricing.

Meeting Times
Classes meet 3 times a week on Monday, Wednesday, and Friday. Hours of operation are 8am-8pm. Patients have the flexibility of coming at any time.

Education Seminars
PROHEALTH strives to educate people on how to make smarter food choices and cook healthier. We offer frequent education classes which include information on meal planning, how to eat right, and more. A nutritional evaluation is included in the monthly fee based on a 3-day intake done by the patient.
